Job Summary
BASIC FUNCTION: The Registered Dietitian Nutritionist (RDN) oversees nutritional care for adult patients with a wide range of health conditions. Visits address the following patient diagnoses and goals (but not limited to): patients with diabetes, weight management goals, cardiovascular disease, hypertension, gastrointestinal symptoms, malnutrition, celiac disease, food allergy needs, home tube feeding regimen calibration, and others.This role will support primary care nutrition referrals from our Chelsea Health Center and our West Ann Arbor Health Center.
The RDN delivers nutrition education and medical nutrition therapy services for prevention, wellness and disease management to help individuals make patient-centered, positive lifestyle changes. RDNs facilitate patient self-management support through patient-specific goal setting and empowerment to achieve and maintain sustainable lifestyle changes. RDNs coordinate with other care managers as needed to refer across the care management spectrum, and work with other team members to provide evidence-based, patient-centered care.

The person who fills this role will be asked to work 24 hours per week.
Work Locations
The RDN in this position will be working from home providing virtual care.

Background Screening
Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings.
