Summary:

The Behavioral Health Resource Coordinator (BHRC) I will provide general assistance, information and referrals for necessary resources and Behavioral Health services. The position will support the BHRC II with helping families access the necessary services for children's mental and behavioral needs.

Description
1. Coordinates referral for families and professionals seeking behavioral health services and resources.
 
2. Takes direction from BHRC II to help families access resources and services.
 
3. Creates and maintains resource database.
 
4. Provides collateral information to BHRC II/III so they may complete psychosocial assessment and any screening and/or triage to assist families in determining necessary services.
 
5. Maintains accurate records of referrals, contacts, and follow-up needs. Records may be used for quality metrics so accuracy is critical.
 
6. Provides updates to BHRC II/III on family's follow-up with referrals, ongoing needs, and compliance with recommendations.
 
7. Performs other duties as assigned.
